USMS Number Pending:
Swimmers will only be entered in the meet with a valid USMS number.

WAIVER SIGNATURE:  Fill out your top-right box (name, ID number, etc.), and sign and date
a new form. Write the words "ENTRY WAIVER ONLY" across the top, and mail it to the
regular address for entries.

ALL PROBLEM ENTRIES MUST BE RESOLVED BY 12/5; swimmer with problems not resolved
by 12/5 will not be swimming the meet.
